Your Initial Visit

A permanent medical record will be initiated upon your first visit to our office.  Please have the following information available for us at that time.

  • An accurate list of all current medications.
  • A list of all allergies.
  • Medical insurance cards.
  • Insurance forms when required.

Insurance

We strive to keep our health care costs reasonable.  We accept most commercial insurance plans and participate in several PPOs and HMOs including:

  • Blue Cross / Blue Shield
  • Medicare
  • Aetna US HealthCare
  • BC/BS PPO and Blue Preferred Plus
  • PPOM
  • SelectCare
  • United Health care
  • DMC
  • AARP
  • American Community

If you have a question about insurance, just call our office during regular business hours.

As a courtesy to our patients, we file primary and secondary insurance with insurance carriers.  Whenever your insurance changes or you are issued a new insurance card by your employer or insurance carrier, we need to update the information in our records.  Please call with the information and bring the card to your next appointment so we can make a copy of it. This will help minimize any delays in processing your claims.


Prescription Refills

It is your responsibility to have sufficient medication on hand at all times.  Your physician will give you enough medication refills to last you until you need to be rechecked.  Please call for an appointment when you have filled your last refill if you don’t have one scheduled.
Have your pharmacy call during regular office hours for prescription refills.


Medical Records

Your medical records are confidential, and no information whatsoever will be given without your consent or written permission, via a record release form.  Charges may be made for statements to attorneys, disability forms, or insurance forms, based on the time involved.
